# Booking forms

**Each unit has a booking form that guests fill in when they book. It asks for their contact details and anything else you need to know, and it can add prices, such as a cleaning fee or an optional breakfast.**

Open a unit and go to _Booking form_. The form has two parts: _Contact details_ and _Booking details_.

## [Contact details](#contact-details)

The fields that describe the guest: name, email address, phone number, address and custom contact details. Guests who book are saved as [contacts](https://www.bookingmood.com/en-US/docs/guest-communication/contacts) with these details.

For each field, choose whether it's _Required_, _Optional_ or _Hidden_, and how it's shown:

- _Name field_: _First + last name_ or a _Single field_.
- _Phone field_: _International_, and optionally _With SMS opt-in_, so guests can agree to receive [SMS messages](https://www.bookingmood.com/en-US/docs/guest-communication/sms).
- _Address field_: _Segmented_ (street, city, ZIP code, country) or a single field.
- _Custom contact detail_: any other detail you want to save with the contact, such as a license plate.

Questions about the stay. Click _+ Field_ and choose a _New field_ or an _Existing field_: fields can be shared by several units, so you only set them up once.

| Field | Description |
| --- | --- |
| _Single-line text field_ | A short answer. |
| _Multi-line text field_ | A longer answer, such as special requests. |
| _Number field_ | A number, with an optional minimum and maximum. |
| _Time field_ | A time, such as the expected arrival time. |
| _Checkbox field_ | Yes or no. Can add a price when checked. |
| _Multiple choice field_ | One of several options, shown as a _Dropdown_ or a _List_. Each option can add a price. |
| _Fee / description_ | Text or a fixed price that always applies, such as a cleaning fee. |

To use a field for one unit only, open it and click _Unlink from unit_. _Copy field_ creates an independent copy.

## [Add fees](#add-fees)

Prices in the booking form work like [rent prices](https://www.bookingmood.com/en-US/docs/rent-and-restrictions/rent-prices): per booking, per night, per guest, split by season and so on. Each fee can also have [taxes](https://www.bookingmood.com/en-US/docs/rent-and-restrictions/taxes), a deposit, discounts and its own payment schedule, see [Deposits, discounts and installments](https://www.bookingmood.com/en-US/docs/booking-form/deposits-discounts-installments).

### [Tourist tax per guest per night](#tourist-tax-per-guest-per-night)

A tourist tax needs no input from the guest and always applies, so use a _Fee / description_ field with a price per night per guest.

Use a _Fee / description_ field with a price per booking: don't select anything after the amount.

Use a _Checkbox field_. The price is only added when the guest checks it.

### [Guests see an empty booking form](#guests-see-an-empty-booking-form)

The form only shows the fields you added. Open the unit's _Booking form_ and add fields to _Contact details_ and _Booking details_.
